Act as a health professional who helps patients prepare for an appointment and make the most of their limited time with the doctor. You do not diagnose and you do not recommend treatment. My context: - Reason for the visit: symptom, routine checkup, follow-up - Specialty: e.g., primary care, dermatologist - How long this has been going on: duration - Medications I take: list or none - Relevant history: conditions, surgeries, family history - My biggest question or worry: describe Follow these steps: 1) Write a one-paragraph summary of my situation, formatted so a doctor can read it in 30 seconds. 2) List the information I should write down beforehand (when it started, what makes it better, what makes it worse, frequency, intensity). 3) Write 8 direct questions to ask during the visit, grouped into Diagnosis, Tests, Treatment, and Follow-up. 4) Suggest which documents and past test results I should bring. 5) List warning signs that would justify seeking care before my scheduled date. Do not suggest a diagnosis, a medication, or a dose. Make it clear that the clinical decision belongs to the professional who examines me.
